Personal licences

To apply for a personal licence, please fill in the following:

• Personal licence application form
• Disclosure of convictions and civil penalties declaration form

Please make sure you have read the information contained in our privacy notice.

• Please then submit these to the address below, together with:
• two passport style photographs, one of which is endorsed as a true likeness of me by a solicitor or notary, a person of standing in the community or any individual with a professional qualification                                         
• a licensing qualification or proof you are a person of prescribed description
• a criminal conviction certificate or a criminal record certificate or the results of a subject access search of the police national computer by the National Identification Service
• an up to date DBS certificate from the Disclosure and Barring Service (at the time you apply your certificate must be no less than one month old)
• proof of right to work in the United Kingdom (read the guidance notes in the application form to find out more)
• the correct payment (please make cheques payable to Lichfield District Council) - view the fees
